Refractory obsessive-compulsive disorder has been a small but high-need population with limited options beyond intensive cognitive behavioural therapy and SSRIs. Deep brain stimulation has approval; transcranial magnetic stimulation now has an OCD-specific indication; psychedelic-assisted therapy programs are in early trials. The competitive question is which modality clears the operational and reimbursement bar for routine use first.
